Enhanced Care Management

Coordinated support for people experiencing homelessness, helping bridge the gap between a single appointment and lasting care by staying involved in the details.

WALNUT OFFICE | STREET MEDICINE OUTREACH

Medical & Street Medicine

A fairly full scope of primary care for people who are uninsured, pregnant, or navigating a challenging situation, plus a Street Medicine team that comes to you for wellness checks and post-hospitalization care.

  • Wound care, vaccines & women’s health
  • Narcan distribution & first aid / basic medication
  • Medication-Assisted Treatment (MAT) for substance use
  • Post-hospital recuperative care & wellness checks (Street Medicine)

Mental Health

Support for children, adolescents, older adults, and families working through mental health, emotional, behavioral, or life-related challenges. Access starts with a referral from a Care Manager or Youth Program team member, reach out through any 4th Second program and we’ll connect you.

  • Behavioral health assessments
  • Individual counseling & crisis intervention
  • Group therapy
  • Linkage to psychiatric & community-based supports

Youth ages 12 and older can access mental health services without a parent or guardian.
